New Patients

Upon your first visit, you will be asked to fill out several forms including patient information, privacy notices, financial policy agreement etc. Please arrive 15 minutes early to allow yourself time to check in and recieve appropriate forms. You will also be asked for a copy of your insurance card, or necessary insurance information so that we may copy for our billing office.

Appointments

Our receptionists are available to assist you between 9:00 am - 12:00 noon, and 2:00-5:00 pm Monday thru Friday. Our phones are busiest between 9:00 and 10:00am. If your call isn't urgent, we encourage you to call after 10:00am, when the phones are not as busy.

Sick children needing appointments will be seen the day you call for an appointment. Because we must leave space for ill children, our Well Child appointments may not be as immediately available. Please schedule your well child exams as far in advance as possible.

If you cannot keep an appointment, please call us at least 24 hrs in advance to cancel or reschedule. A service charge may be applied to missed visits. Please consider a missed visit leaves a vacant space that could have allowed another child to be seen.

When calling, be prepared to write down instructions or advice you are given. It is easy to forget or misunderstand instructions. Have the telephone number and hours for your nearby pharmacy in the event we need to call in medication. Take your child's temperature before calling. Knowing the accurate temperature is essential for our staff to be able to assist you properly.
